Please ensure that the patient understands that, by ordering their free testing, the patient gives you permission to contact them to share test results and updates and that if they test positive for any test, they give you permission to share your test results with another clinician who will advise on treatment.

Patient Login Username and Password

Because you have not entered an email or phone number, the patient record requires a username and password. You do not have to provide this information to the patient but the account must still be password protected.
